What is the main reason for putting your membership on hold?*

By Submitting below I understand all hold requests must be submitted at least 14 days in advance. Submitting a hold request within 14 days will not be processed and we ask that you refrain from emailing us unless it is an extreme circumstance. (Please note there is a maximum hold time for membership of 4 weeks per annum.

Anything more (excluding injury) is considered a cancellation, which will then require you to please complete our cancellation form instead). If more then 4 weeks are required due to holidays and/or other reasoning there will be a $5 recurring freeze fee that will apply.

IMPORTANT: Please complete all required fields to ensure your freeze request is processed accurately. Incomplete forms may delay processing or may not be processed at all.
